This co-ed program will be located in Chester, New Jersey and will serve up to five youth, ages 13 to 18 years old, with behavioral/emotional challenges due to complex trauma who need emergency short term out of home placement.
The Program Transition Specialist is responsible for case management for all the youth in the assigned home.
The Program Transition Specialist will work closely with the youth, caregivers, school districts, CMO's and other outside agencies to coordinate services as indicated by the treatment team.
As a member of each youth's treatment team, this position focuses heavily on coordinating services as indicated for the youth throughout their stay in the program up to and including discharge planning.
Documents progress on each youth on a weekly basis.
Assists the Program Manager in all of their duties including oversight of the staff, quality assurance/performance improvement activities, scheduling, etc.
The Program Transition Specialist is a functional member of the program's Leadership Team and is expected to carry out certain Child Care Counselor duties as needed.
Candidate will possess the following:
Education /
Experience:
Bachelor's Degree in a Mental Health related field with 3 to 5 years relevant experience or an unlicensed Master's level practitioner with one-year relevant experience.
Valid Driver's license - not currently suspended or suspended over the last three years due to accidents or moving violations.
